Most supported living placements are funded by the local authority (council) following a care and support assessment. If you have eligible care needs under the Care Act 2014, the council may fund all or part of your support.

NHS Continuing Healthcare (CHC)

Fully funded NHS care for complex health needs

NHS Continuing Healthcare (CHC) is a package of care arranged and funded entirely by the NHS. It is available to adults who have complex health needs arising from disability, injury or illness that go beyond what a local authority can meet.

  2. 2Multi-disciplinary team assessment (MDT) carried out
  3. 3Decision Support Tool (DST) used to assess eligibility
  4. 4Eligibility determined by Integrated Care Board (ICB)
